BACKGROUNDAcne vulgaris is a common chronic inflammatory disease of the pilosebaceous unit, affecting approximately 9.4% of the global population, particularly adolescents and young adults. It is characterized by seborrhea, comedones, inflammatory lesions, and, in some cases, complications such as acne-induced hyperpigmentation, acne-induced erythema, and scarring. Beyond its physical manifestations, acne can substantially impair quality of life and is associated with psychological distress. Many individuals initially seek advice from community pharmacists and self-manage with over-the-counter (OTC) treatments. Topical therapies remain the cornerstone of treatment, while moderate to severe cases may require systemic therapies. International consensus recommendations support the use of dermocosmetics, including cleansers, moisturizers, and sun protection, to improve treatment tolerability, adherence, quality of life, and long-term outcomes.Objective To develop a practical care algorithm outlining the role of community pharmacists in the management of acne vulgaris. Methods A literature review on pharmacists’ roles in dermatological care and current acne management guidelines was conducted in December 2025. An international online survey of FIP members was carried out between March and April 2026 to assess current practice, knowledge, and perceived barriers to pharmacist involvement in skin health. A total of 142 responses from 51 countries were analyzed. Based on the literature review and survey findings, an expert FIP panel of 13 pharmacists from five continents recommended the development of practical guidance on acne and other skin conditions. A pharmacist-centred care algorithm for acne management was subsequently developed, reviewed with the assistance of the International League of Dermatological Societies (ILDS) and supported by the International Pharmaceutical Federation (FIP). Results The algorithm identifies key pharmacist interventions across the patient journey. As an accessible first point of contact, pharmacists can assess acne severity and lesion characteristics, identify patients suitable for OTC management, and recognize referral criteria. Pharmacists can support the selection and appropriate use of acne treatments and dermocosmetics, promote healthy skincare practices, and provide advice on lifestyle measures. For patients receiving prescribed therapies, pharmacists can counsel on expected outcomes and adverse effects, monitor treatment use, support adherence, and recommend adjunctive dermocosmetic products to manage treatment-related dryness and irritation. Pharmacists also contribute to antimicrobial stewardship, safe isotretinoin use, and referral when psychosocial or clinical concerns are identified. During maintenance phases, dermocosmetics and photoprotection may help reduce relapse and acne-induced hyperpigmentation. Conclusion Pharmacists are often the first point of contact for people with acne and can play an important role in its management through assessment, first line treatment recommendation, dermocosmetic recommendations, antimicrobial stewardship, and referral when needed. Pharmacist-led interventions in community pharmacy settings can improve adherence, quality of life, and clinical outcomes.
