CHRONIC RADIATION DERMATITIS

TYPE OF TREATMENT PROACTIVE
(PREVENTION/STEP 0)
REACTIVE
(MANAGEMENT IN CASE OF AE)
  • Radiation therapy, enhanced by concomitant chemotherapy or targeted therapy (especially 5-fluorouracil and EGFR inhibitors) but not necessarily by checkpoint inhibitors.
  • Proper radiation therapy techniques
  • Gentle cleanser with pH ~5
  • Moisturizer
  • Sun protection
  • Gentle skin care
  • Educate patient about risk of late development of skin cancers in treated areas
  • Thermal water
  • Thermal cure
  • Pentoxifylline 400 mg TID to prevent radiation induced fibrosis (in later part of radiation therapy or following development of fibrosis; use for at least 6 months as long as it is still improving the condition)
  • Vitamin E (use with caution in patients taking anticoagulant or antiplatelet therapy)
